Abstract

The obtained data have demonstrated that in patients with ischemic heart disease (IHD) the destabilization in their condition was marked during the first 7—15 days of COPD exacerbation, especially of viral ethiology. The most often sings of IHD exacerbatons were different arrythmias (57,3 %), including 16,2 % registered for the first time in life. Besides this, instable forms of stenocardia and miocardial infarction can occur. Thus, the treatment of these patients must include not only antiviral and antimicrobic drugs, but antianginal and antiarythmic medications, including betal-blockers for treatment of patients with hyperkinetic syndrome as well.

Abstract

The article displays the results of endobronchial C02-laser phototherapy for treatment of nonspecific bronchitis in 23 patients with tuberculosis and 24 patients with COPD. It is estimated that C02 phototherapy results in the approvement of cytogram and simultneous involution of bronchial pathology. The degree of BAL fluid cytogram normalization during the recovery from nonspecific bronchitis in case of tuberculosis is determined by its form, and in case of COPD — by the intensity and the degree of irreversibility of pathologic changes.

Abstract

X-ray data of 80 patients with different purulent lung processes were analysed. Three groups of patients with acute infectious lung destructions were singled out.

In the first group the purulence was caused by anaerobes and their association with other bacteria. In the second — gram-negative organisms and their associations with other bacteria, and in the third — staphylococcus was the main cause of purulent process. The analysis of X-ray data did not reveal any significant features which can be used as a differential diagnostic criteria of different microbic factors. Thus, the exact ethiologic diagnosis of purulent lung process based on X-ray data is impossible.

Abstract

49 lung cancer cases with different histologic forms were investigated. The study was performed with the help of cryosta-tic slices with minimal number of artifacts, induced by disguise and destruction of tissue antigens during their histological work-up. The distribution and correlation between the main structural components of basal membranes (BM) — type 4 collagen, laminin and heparansulfateproteoglycane — were studied. It was established that these markers take part in the forming of lung cancer BM. Exept for carcinoid tumors and zones of desmoplasia, the dissociation of markers bears microstructural character. The type 4 collagen, laminin and heparansulfateproteoglycane to a greater extend are elicited in desmoplasia zones and intercellular matrix. The degree of lung cancer BM expression does not depend on the histological form and the level of tissue differentiation of the tumor. That investigation testifies about the different variants of tumor invasion, regardless of its histologic form, the level of differentiation and localization in central or peripheral parts of the lung.

Abstract

The article deals with the investigation of cough and mico-cilliary clearence in patients with nonspecific iung diseases (lobular pneumonia, lung abscess, intrinsic asthma). The aim of this study is to define quantitative criteria of cough efficacy which in terms give the integral characteristic of tracheobronchial clearence components. It is achieved by monitoring the time of inuline expectoration and registering of cough jolts with the help of tussograph IKT-1. In case of prolongation of inuline expectoration time using drug suppression of cough, the latter is regarded effective.

Abstract

Extracorporal Immunopharmacopherapy (EIPT) with Prednisolone and Ciancobalamini (В 12) is a method of the treatment of severe glucocorticosteroid (GCS) — dependent atopic bronchial asthma. •

Leucocytes 4—8 billions collected to the collection-bag by means of blood cell separator, incubated with Prednisolone 30 mcg/ml and B12 1 mcg/ml for 3 houres at 37 °C, washed with sodium solution 3 times and then reinfused to patients. A therapeutic effect is isually reached after 3—5 procedures repeated every 3—5 days.

We put EIPT into practice for treatment of 20 patients with atopic atshma. All that patients had got GCS-dependence before EIPT and exhibited evedence for remition for 6 to 24 months without sistem GCS after EIPT. Moreover these patients showed decreasment of histamine skin sensitivity and IgE-levels in plasma, elevation of plasma cortisol concentration. These patients mononuclear cells had got high level of histamine liberation activity before EIPT and had decreasment it after treatment.

EIPT with Prednisolone and B12 appears to be an effective method for abolishing of GCS-dependence in patients with severe GCS-dependent atopic asthma and preventing severe complications of GCS-treatment.

Abstract

The determination of surfase lymphocyte antigens in patients with atopic bronchial asthma has been carried out using the monoclonal antibodies of the home series LT. The examined patients have been found to have some disorders of the expression antigens under investigation on immunoregulating cells. These disorders are more strongly marked during acute periods of the disease. The decrease of the expression of CD8 antigen is typical of bronchial asthma patients, the amount of CD8+ lymphocytes being 15,6=b0,3 % in the acute period and 17,04hl,8 % during period of remission against 23,84=0,6 % in healthy donors.

The results obtained confirm the opinion about insufficiency of supressor T-lymphocytes in the cases of atopic bronchial asthma and enable to recommend the determination of surface lymphocyte antigens as one of the immunological criteria when this disease takes play.

Abstract

We present the method bronchoscopic endoprosthetics of trachea and main stem bronchi with the help of inserting bifurcation stent for liquidation of severe stenosis caused by endophitic cancer. Manipulation was performed after laser photodestruction of tumoural exophitic part under general anaesthesia with high frequent artificial ventilation of lung over thin catheter, introduced into the main stem bronchus.
