Целью исследования явилась оценка распространенности текущих астмоподобных симптомов (тАПС) и аллергического ринита (тАР) у детей 3–6 лет и установление факторов риска их развития.

Материалы и методы

Материалы и методы. В 5 городах Алтайского края проводилось кросс-секционное исследование (2015–2016). Оценка распространенности тАПС и тАР проводилась с помощью русифицированной версии опросника Международного исследования астмы и аллергии у детей (International Study of Asthma and Allergiesin Childhood – ISAAC). Факторы риска выявлялись с помощью дополнительного опросника, заполняемого родителями.

Результаты

Результаты. Заполнено 3 205 опросников, по результатам анализа которых установлено, что распространенность тАПС составила 11,1 %, тАР – 18,0 %. Из 3 205 детей аллергический ринит (АР) верифицирован врачами у 6,4 % детей, бронхиальная астма – лишь 0,9 %. При отягощенном семейном анамнезе по аллергическим заболеваниям (АЗ) риск развития тАПС в дошкольном возрасте повышается в 2 раза (ОШ – 2,11, 95%-ный доверительный интервал (ДИ) – 1,66–2,68; p < 0,01), тАР – в 2,6 раза (ОШ – 2,63; 95%-ный ДИ – 2,16–3,19; p < 0,01). В случае принадлежности ребенка к мужскому полу риск развития тАПС повышается в 2,6 раза (ОШ – 2,63;95%-ный ДИ – 1,17–5,93; p < 0,01), тАР – в 1,3 раза (ОШ – 1,35; 95%-ный ДИ – 1,01–1,37; p < 0,05). В случае курения родителей на 1-м году жизни ребенка риск развития тАПС повышается в 1,6 раза (ОШ – 1,61; 95%-ный ДИ – 1,15–2,24; p < 0,01).

Заключение

Заключение. По результатам анализа установлено, что распространенность тАПС и тАР значительно превосходит таковые показатели при верификации указанных диагнозов врачами. Показано, что отягощенный по АЗ семейный анамнез, мужской пол и курение родителей на 1-м году жизни ребенка являются значимыми фактором риска развития тАПС и тАР в дошкольном возрасте.

 

 

The objectives of this study were to estimate prevalence of asthma-like symptomsand allergic rhinitis in preschool children and to determine risk factors of these diseases.

Methods

Methods. This cross-sectional study was conducted in 5 cities of Altai krai in 2015 – 2016 and included children aged 3 to 6 years. Prevalence of allergic diseases was assessed using the Russian version of the ISAAC questionnaire.

Results

Results. The prevalence of asthma-like symptoms was 11.1%, and the prevalence of allergic rhinitis was 18.0%. The diagnoses of asthma and allergic rhinitis were physician-confirmed in 0.9% and 6.4% of 3205 children, respectively. Family history of allergic diseases was related to the increased risk of asthma-like symptoms (ОR 2.11; 95% CI 1.66 – 2.68) and allergic rhinitis (ОR 2.63; 95% CI 2.16 – 3.19). Boys also had the increases risk of asthma-like symptoms (OR 2.63; 95% CI 1.17 – 5.93) and allergic rhinitis (ОR 1.35; 95% CI 1.01 – 1.37). Smoking in parents during the first year of life of the child increased the risk of asthma-like symptoms (ОR 1.61; 95% CI 1.15 – 2.24).

Conclusion

Conclusion. The prevalence of asthma-like symptoms and allergic rhinitis considerably exceeds the rate of physician-confirmed diagnosis. Family history of allergic diseases, male gender, and smoking in parents increased the risk of allergic diseases in preschool children.
