Целью исследования являлось определение на основе анализа современной литературы и собственного клинического опыта места цефтобипрола в лечении пациентов с внебольничными (ВП) и нозокомиальными (НКП) пневмониями в условиях роста антибактериальной резистентности и постинфекционных иммунных нарушений. Результаты. По данным клинического наблюдения № 1 продемонстрирована эффективность эмпирической терапии цефтобипролом у госпитализированной пациентки с ВП и признаками деструкции легочной ткани, клиническими предикторами «проблемных» возбудителей, анамнестическими указаниями на неэффективность стартовых режимов антибактериальной терапии (АБТ) на догоспитальном этапе. По данным клинического наблюдения № 2 отмечено успешное применение препарата у пациента с тяжелым неврологическим дефицитом, при котором потребовались трахеостомия и гастростомия, рецидивирующими нозокомиальными инфекциями нижних дыхательных путей, ассоциированными с панрезистентной грамотрицательной флорой и исчерпанием возможностей различных вариантов АБТ. Цефтобипрол включен в утвержденные в лечебном учреждении внутренние протоколы эмпирической АБТ при лечении ВП и ранних НКП, накоплен широкий опыт применения препарата у пациентов этой категории. Заключение. Рекомендуется масштабирование представленного опыта по применению цефтобипрола у пациентов с ВП при риске «проблемных» возбудителей. Приведенный в клиническом примере № 2 подход можно рассматривать в сложных клинических ситуациях, ассоциированных с исчерпанием возможностей АБТ, однако при этом требуются дополнительные исследования.</p></abstract><trans-abstract xml:lang="en"><p>Ceftobiprole medocaril is a fifth-generation cephalosporin antibiotic with expanded activity against Gram-positive bacteria, including methicillin-resistant Staphylococcus aureus, and the activity against Gram-negative bacteria characteristic of third- and fourth-generation cephalosporins. The aim was to determine the role of ceftobiprole medocaril in the treatment of patients with community-acquired and nosocomial pneumonia in the context of increasing antibiotic resistance and post-infectious immune disorders based on an analysis of current literature and our own clinical experience. Results. The first clinical case demonstrates the efficacy of empirical ceftobiprole therapy in a hospitalized patient with community-acquired pneumonia and signs of lung tissue destruction, clinical predictors of challenging pathogens, and predictors of the ineffectiveness of initial antibacterial regimens in the prehospital setting. The second case demonstrates the successful use of the drug in a patient with severe neurological deficit requiring tracheostomy and gastrostomy, recurrent nosocomial lower respiratory tract infections associated with pan-resistant gram-negative bacteria, and exhaustion of various antibacterial treatment options. Ceftobiprole medocaril is included in our clinic’s internal empirical antibacterial therapy protocols for community-acquired and early nosocomial pneumonia, and extensive experience has been accumulated in this patient population. Conclusion. We recommend scaling up our experience in using ceftobiprole in patients with community-acquired pneumonia and the risk of challenging pathogens.
