В последние годы появились новые подходы к эпидемиологии, диагностике, классификации (категоризации), оценке фенотипов, а также характеристике и оценке тяжести обострений ХОБЛ. Существенно изменились современные подходы к стартовой и последующей медикаментозной терапии. Это во многом связано с результатами проведенных в последние годы крупных исследований, по данным которых продемонстрирована высокая эффективность тройных фиксированных комбинаций, включающих ингаляционные глюкокортикостероиды, длительно действующие β-агонисты и антихолинергические препараты.</p><p>Целью обзора явилось освещение последних достижений в лечении ХОБЛ с учетом нюансов патофизиологии ее подтипов, которые будут иметь решающее значение для уменьшения симптомов заболевания. В связи с  появлением новых препаратов возникала необходимость в модификации отечественных алгоритмов ведения пациентов с ХОБЛ.</p><sec><title>Методы</title><p>Методы. Впервые в клинические рекомендации включен биологический препарат дупилумаб, который назначается пациентам с ХОБЛ с признаками воспаления 2-го типа и симптомами хронического бронхита. Важными остаются вопросы использования немедикаментозных методов терапии (отказ от курения, физическая активность и дыхательная реабилитация), современные подходы к лечению дыхательной недостаточности.</p></sec><sec><title>Заключение</title><p>Заключение. Согласно новым дополнениям к клиническим рекомендациям подчеркивается важность раннего вмешательства, персонализированного лечения с учетом разнообразия фенотипов ХОБЛ, заложена основа для более эффективной и индивидуализированной модели лечения пациентов с ХОБЛ. Благодаря способности контролировать воспаление и улучшать функцию легких у пациентов с ХОБЛ с помощью современных подходов к терапии следует ожидать не только улучшения качества жизни пациентов, но и потенциального снижения общей нагрузки на систему здравоохранения.</p></sec></abstract><trans-abstract xml:lang="en"><p>Chronic obstructive pulmonary disease (COPD) is a common respiratory disease, a leading cause of morbidity and mortality worldwide. In recent years, new approaches to the epidemiology, diagnosis, classification (categorization), phenotype assessment, and characterization and severity of exacerbations of COPD have emerged. Modern approaches to initial and subsequent drug therapy have changed significantly. This is largely due to the results of large studies conducted in recent years, demonstrating the high efficacy of triple fixed-dose combinations including inhaled glucocorticosteroids, long-acting β-agonists, and long-acting anticholinergics.</p><p>The aim  of this review was to highlight the latest advances in COPD treatment, taking into account the nuances of the pathophysiology of its subtypes, which will be crucial for reducing the symptoms of the disease. Due to the emergence of new drugs, there was a need to modify domestic algorithms for managing patients with COPD.</p><sec><title>Methods</title><p>Methods. For the first time, the biological agent dupilumab has been included in clinical guidelines; it is indicated for patients with COPD with signs of type 2 inflammation and symptoms of chronic bronchitis. The use of non-pharmacological therapies (smoking cessation, physical activity, and respiratory rehabilitation) and modern approaches to treating respiratory failure remain important.</p></sec><sec><title>Conclusion</title><p>Conclusion. New additions to clinical guidelines emphasize the importance of early intervention, personalized treatment, and the diversity of COPD phenotypes, and also lay the foundation for a more effective and individualized COPD treatment model.
