Кашель – одна из серьезных проблем практической пульмонологии. Механизмы возникновения кашля разнообразны, а заболевания, протекающие с хроническим кашлевым синдромом, многочисленны. Сложна диагностическая ситуация, связанная с «непонятным», «необъяснимым», «идиопатическим» кашлем.

Целью настоящего обзора явилось рассмотрение основных механизмов формирования «непонятного» кашля, вариантов течения, алгоритма ведения пациентов с трудноизлечимым кашлем, а также новых методов лечения.

Заключение. Хронический «непонятный» кашель нарушает качество жизни пациентов, снижает их работоспособность. Знание механизмов его возникновения, способствует повышению эффективности терапии.

Cough is a serious problem of practical pulmonology. The mechanisms of cough occurrence are diverse and numerous diseases are associated with chronic cough syndrome. The diagnosis can be challenging because of a “perplexing”, “unexplained”, or “idiopathic” cough.

The aim of the review was to consider the main underlying mechanisms of the “perplexing” cough, its potential course, an algorithm for managing patients with the persistent cough, and new treatment methods.

Conclusion. Chronic “perplexing” cough disrupts the patient’s quality of life and reduces their performance. Unveiling the mechanisms of its development may increase the treatment efficacy.
