Приведены результаты анализа 62 научных работ, опубликованных за 23 года (2001–2023), по проблемам изучения митохондриальной дисфункции (МД), новых методов ее диагностики и путей коррекции при заболеваниях органов дыхания. В качестве актуальной научной проблемы респираторной медицины определено нарушение энергетической функции митохондрий. Отмечено, что для решения указанной проблемы разрабатываются новые методы патогенетической терапии при распространенных и труднокурабельных в настоящее время заболеваниях. Ключевой раздел обзора посвящен результатам исследований нарушений функций митохондрий у больных бронхиальной астмой, хронической обструктивной болезнью легких, идиопатическим легочным фиброзом и легочной гипертензией.</p><p>Заключение. Представлена оценка методов диагностики МД, потенциально применимых в условиях реальной клинической практики. Проведено сопоставление результатов изучения электрического потенциала внутренней мембраны митохондрии, накопления активных форм кислорода, продукции аденозинтрифосфата в клетках крови, альвеоцитах и эпителии дыхательных путей, а также гладкомышечных клетках (ГМК) бронхов, эндотелии и ГМК сосудов системы легочной артерии, определения митохондриальной ДНК в биологических cредах. Кратко освещаются отдельные предложения для коррекции синдрома МД в клинической картине респираторных заболеваний.</p></abstract><trans-abstract xml:lang="en"><p>The aim of this scientific literature review was to find new solutions to urgent problems of treating respiratory diseases and reducing the associated temporary loss of ability to work and disability. The article presents the results of the analysis of 62 scientific papers published over 23 years (2001 – 2023) on mitochondrial dysfunction (MD), new methods of its diagnosis, and ways of its correction in respiratory diseases. Disorder of the energy function of mitochondria has been identified as an urgent scientific problem of respiratory medicine. It is noted that new methods of pathogenetic therapy for common and currently difficult-to-treat diseases are being developed to solve this problem. The key section of the review is devoted to the results of studies of mitochondrial dysfunction in patients with asthma, chronic obstructive pulmonary disease, idiopathic pulmonary fibrosis, and pulmonary hypertension.</p><p>Conclusion. The article assesses MD diagnostic methods that are potentially applicable in real clinical practice. The results of studying the electric potential of the inner mitochondrial membrane, accumulation of active forms of oxygen, production of adenosine triphosphate in blood cells, alveocytes and epithelium of the respiratory tract, as well as bronchial smooth muscle cells, endothelium and vascular smooth muscle cells in the pulmonary artery system, determination of mitochondrial DNA in biological environments are compared.
