Ведение РЛ, ассоциированного с беременностью, представляет собой клиническую дилемму, поскольку необходимо учитывать возможные риски в отношении не только матери, но и плода. Целью работы явилось изучение существующих литературных данных о диагностике, тактике ведения и лечебных подходах при РЛ, ассоциированном с беременностью. По данным литературных источников и представленных клинических наблюдений показаны неутешительные результаты: практически не наблюдалось ответа на химиотерапию (ХТ) у пациенток, получавших ХТ комбинациями платины с винорелбином, паклитакселом и этопозидом во время беременности и после родов, при этом сделан предварительный вывод о том, что изменение фармакокинетики химиопрепаратов может поставить под угрозу лечение онкологического заболевания во время беременности. Применение хирургического и лучевого методов лечения зависит от конкретной клинической ситуации, стадии и гистотипа опухоли, а также срока беременности. Заключение. Таким образом, как диагностические, так и лечебные мероприятия в отношении беременных женщин с РЛ должны быть персонализированными и выполняться специальной мультидисциплинарной командой, включающей в себя химиотерапевта, радиотерапевта, онколога-хирурга и акушера-гинеколога.</p></abstract><trans-abstract xml:lang="en"><p>According to world statistics, lung cancer (LC) ranks 2nd in the cancer incidence among women. Management of pregnancy-associated LC presents a clinical dilemma, as it is necessary to consider possible risks not only for the mother, but also for the fetus. The aim of the work was to study the existing literature data on diagnosis, management tactics and treatment approaches for pregnancy-associated cancer. The literature sources and presented clinical observations show disappointing findings: there was practically no response to chemotherapy (CT) in patients who received chemotherapy with platinum in combination with vinorelbine, paclitaxel, and etoposide during pregnancy and after childbirth, and a preliminary conclusion was made that change in the pharmacokinetics of chemotherapy drug may jeopardize the treatment of cancer during pregnancy. The use of surgical and radiation treatment methods depends on the individual clinical situation, stage and histotype of the tumor, as well as the gestational age. Conclusion.
