Мукоцилиарный клиренс (МЦК) и мышечная система легких являются теми защитными механизмами, которые обеспечивают успешную работу респираторной системы. При патологии легких, приводящей к дыхательной недостаточности, происходят патологические изменения в системе МЦК и изменения в дыхательной мускулатуре, в первую очередь связанные с гиперинфляцией. Традиционная медикаментозная терапия не всегда эффективно воздействует на данные процессы. Применение аппарата высокочастотной осцилляции грудной клетки может влиять на очистку дыхательных путей и гиперинфляцию легких. Целью данного исследования служило изучение влияния методов высокочастотной осцилляции грудной клетки на функциональное состояние легких у больных с легочной патологией. В простом открытом сравнительном исследовании участвовали 3 группы. 1-ю группу составили 10 пациентов с бронхоэктатической болезнью (средний возраст – 55,5 ± 39,0 года), 2-ю – 11 больных хронической обструктивной болезнью легких – ХОБЛ (66,2 ± 7,0 года). В первых 2 группах кроме стандартной медикаментозной терапии больным проводилось лечение с помощью вибрационно-компрессионного аппарата Vest. 3-я группа, состоящая из 12 пациентов с ХОБЛ (60,4 ± 8,6 лет), являлась группой контроля и находилась только на традиционной медикаментозной терапии. На 1-м и последнем визитах у больных проводилось исследование функции легких методом спирометрии и бодиплетизмографии и исследование одышки, кашля и продукции мокроты при помощи модернизированной шкалы MRC. В результате проведенного исследования в группах с терапией аппаратом Vest было получено достоверное улучшение функции легких (объема форсированного выдоха за 1-ю с (ОФВ1) – на 9,9 % и 5,7 %, форсированной жизненной емкости легких (ФЖЕЛ) – на 7,7 % и 5 % – в 1-й и во 2-й группах соответственно) и снижение гиперинфляции (остаточного объема легких (ООЛ) – на 4,6 % и 10 % – в 1-й и во 2-й группах соответственно). В группе, где применялась только медикаментозная терапия, достоверно улучшилась лишь ФЖЕЛ – на 3 %. Одышка, кашель и продукция мокроты так же наиболее значимо изменились в группах с терапией аппаратом Vest.</p><p>Таким образом, применение методов вибрационного и компрессионного воздействия на грудную клетку влияет на МЦК и гиперинфляцию легких.</p></abstract><trans-abstract xml:lang="en"><p>Summary. Mucociliary clearance (MCC) and thoracic muscles are defensive mechanisms which provide effective functioning of respiratory system. Usually, respiratory failure is associated with disorders of MCC and respiratory muscles, primarily due to hyperinflation of the lungs. Medication therapy can not successfully improve these changes. Chest high-frequency oscillation could enhance the airway clearance and decrease lung hyperinflation. The aim of this study was to investigate effects of the chest high-frequency oscillation methods on functional status of patients with pulmonary diseases. This open comparative study involved 3 groups of patients: 10 patient with bronchiectasis (the 1st group; mean age, 55.5 ± 39.0 years), 11 patients with COPD (the 2nd group, 66.2 ± 7.0 years), and 12 patients with COPD as controls (the 3rd group, 60.4 ± 8.6 years). Apart from the standard medication therapy, the 1st and the 2nd groups were treated with chest high-frequency compression device VEST, the 3rd group was administered standard drug treatment only. Lung function using spirometry and body plethysmography, dyspnea, cough and sputum production using the modified MRC scale were measured during the first and the final visits. The treatment with VEST device led to significant improvement in lung function (FEV1 increased by 9.9 % and 5.7 % in the 1st and the 2nd groups, respectively, FVC increased by 7.7 % and 5 %, respectively, RV decreased by 4.6 % and 10 %, respectively). In the 3rd group patients, significant improvement was noted only for FVC by 3 %. Also, dyspnea, cough and sputum production improved more markedly in groups treated with VEST device.
