Целью работы является индексная оценка операционных свойств вентиляционных показателей у больных бронхиальной астмой вместо широко распространенного метода сравнения средних. Все обследованные пациенты на основе референтного анализа разделены на 2 группы: больные БА (284 человека) и здоровые (группа сравнения) 101 человек.Объективному решению поставленной задачи способствовало деление больных БА по ОФВ1 % на 4 стандартные группы в зависимости от тяжести функциональных расстройств, что позволило оценить связь между диагностической эффективностью показателей вентиляции у больных БА и степенью вентиляционных нарушений у них. Во всех группах больных использовали стандартные точки разделения — 80, 100, 120%.По результатам исследования проведено ранжирование вентиляционных показателей по степени диагностической валидности.Исследования показали, что оценка диагностической эффективности показателей вентиляции легких у больных БА методами доказательной медицины имеет преимущества по сравнению с традиционной оценкой средних, поскольку позволяет "взвесить" информативность каждой величины с точки зрения вероятности, устанавливает адекватный уровень разграничения нормы и патологии, способствует уменьшению ошибок в функциональной диагностике бронхообструктивных нарушений.
This study was aimed to indexing assessment of operating properties of ventilation parameters which were proposed to be used instead of the widespread method of comparison of the means in bronchial asthma (BA) patients. All the patients examined were divided to 2 groups based on the referent analysis data: 284 BA patients and 101 healthy persons which were the control group.An objective decision of the current problem was provided by a division of all the patients to 4 groups according to severity of their functional disorders. This approach allowed to evaluate a correlation between the diagnostic value of the ventilation parameters and the ventilation disorders severity in the BA patients. The typical cut-points (80, 100, and 120%) were used in all the groups.A ranging of the ventilation parameters according to their diagnostic validity was performed based on the study results.The study showed that the assessment of the diagnostic value of the ventilation parameters in BA patients using the evidence-based medicine methods comes over the typical evaluation of the means as soon as it permits to determine the informative values of all the parameters, to distinguish adequately the health and the pathology, to decrease a number of functional diagnostic mistakes for bronchoobstructive disorders.
