Данные литературы об эффективности использования нового внутрикожного теста с аллергеном туберкулезным рекомбинантным (АТР) Диаскинтест® при диссеминированных процессах в легких единичны и противоречивы.</p><p>Целью исследования явилось изучение диагностических возможностей внутрикожного теста с АТР при диссеминированных процессах в легких различной этиологии.</p><sec><title>Материалы и методы</title><p>Материалы и методы. По данным сравнительного анализа архивных материалов пациентов (n = 204: 85 (41,67 %) женщин, 119 (58,33 %) мужчин; возраст – 19–87 лет) с диссеминированными процессами в легких различной этиологии, поступивших в диагностические отделения Казенного учреждения здравоохранения Воронежской области «Воронежский областной клинический противотуберкулезный диспансер им Н.С.Похвисневой» (2011–2019), изучены данные иммунологического обследования с помощью внутрикожных проб Манту с 2 ТЕ очищенным туберкулином в модификации Линниковой (ППД-Л) и АТР.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что при диссеминированных туберкулезных процессах в легких доля лиц, положительно реагирующих на пробу Манту, была больше только по сравнению с таковой при саркоидозе (p &lt; 0,01) без существенных различий с результатами, полученными у больных других групп (p &gt; 0,05). В то же время доля пациентов, положительно реагирующих на пробу с АТР, при туберкулезе была больше таковой при пневмонии и саркоидозе (p &lt; 0,01) и в некоторой степени – опухолевых процессах. Отмечено также, что доля здоровых лиц с положительной реакцией на АТР по сравнению с таковой при проведении пробы Манту была меньше (p &lt; 0,01). При активном диссеминированном туберкулезе легких отмечены гиперергические реакции (11,9 %) на пробу с АТР, не наблюдавшиеся при пробе Манту. При неспецифических процессах в легких на пробу с АТР реже выявлялись положительные, но чаще – отрицательные реакции (p &lt; 0,05) по сравнению с таковыми на пробу Манту с 2 ТЕ.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные данные свидетельствуют о целесообразности использования пробы с АТР в дифференциальной диагностике туберкулеза и неспецифических заболеваний легких у взрослых пациентов при рентгенологическом синдроме диссеминации.</p></sec></abstract><trans-abstract xml:lang="en"><p>Diseases that manifest as Х-ray syndrome of pulmonary dissemination are one of the most complex differential diagnostic problems in pulmonology and Phthisiology. Literature data on the effectiveness of using a new intradermal test with tuberculosis recombinant allergen (ATR, Diaskintest®) for disseminated processes in the lungs are single and contradictory.</p><sec><title>The aim</title><p>The aim. Тo study the diagnostic capabilities of intradermal test with tuberculosis recombinant allergen in disseminated lung processes of various etiologies.</p></sec><sec><title>Methods</title><p>Methods. The analysis of archival materials of 204 patients with disseminated lung processes of various etiologies who were admitted to diagnostic departments of the N.S.Pokhvisneva Voronezh regional anti-tuberculosis dispensary in 2011 – 2019 was carried out. There were 85 (41.67%) women and 119 (58.33%) men aged 19 to 87 years. The data of im mu no - logical examination of patients using intradermal Mantoux samples with 2 TE PPD-L and with ATR were studied, and their comparative analysis was performed.</p></sec><sec><title>Results</title><p>Results. In disseminated pulmonary processes in tuberculosis, the proportion of individuals who respond positively to the Mantoux test was greater only in comparison with the group of people with sarcoidosis (p &lt; 0.01) and did not significantly differ from the results in other groups of patients (p &gt; 0.05). At the same time, the proportion of people who respond positively to a test with ATR in tuberculosis was greater than in patients with pneumonia, sarcoidosis (p &lt; 0.01) and, to some extent, with a tumor. In healthy individuals, a lower proportion of positive reactions to ATR was established compared to the Mantoux test (p &lt; 0.01). With active disseminated pulmonary tuberculosis, a hyperergic reaction (11.9%) was observed in the sample with ATR, which was not observed with the Mantoux test. In non-specific processes in the lungs, less often than negative reactions (p &lt; 0.05) were detected on the sample with ATR less often than on the Mantoux test with 2 TE.</p></sec><sec><title>Conclusion</title><p>Conclusion.
