Хроническая обструктивная болезнь легких — одна из важных проблем здравоохранения. Основной причиной утраты трудоспособности и смертности больных ХОБЛ является декомпенсация "легочного сердца", в формировании и прогрессировании которого немаловажное значение имеет состояние функции эндотелия. Сведения о влиянии ингаляционных глюкокортикостероидов на функциональную активность эндотелия и нарушения легочно-сердечной гемодинамики при ХОБЛ отсутствуют. Изучена динамика эндотелий-зависимой вазодилятации, уровня циркулирующего эндотелина-1, показателей центрального и легочного кровообращения у 42 больных ХОБЛ 2-й и 3-й стадий на фоне противовоспалительной терапии беклометазоном дипропионатом. У 66 % обследованных выявлены признаки системной дисфункции эндотелия. Показано снижение уровня эндотелина-1 и нормализация эндотелийзависимой вазодилатации после трехмесячного курса лечения. Зарегистрировано снижение систолического давления в легочной артерии и улучшение диастолической функции миокарда правого желудочка.

Chronic obstructive pulmonary disease (COPD) is one of important problems of public healthcare. The aim of the present study was to investigate effect of beclomethazone on endothelial function (endothelin-1 level and results of Celermajer's test) and ultrasound parameters of pulmonary and central circulation in 42 COPD patients. Endothelium dysfunction was found in 66% of patients. It improved after antiinflammatory treatment. The improvement in myocardial diastolic function and decrease in systolic pulmonary artery pressure were revealed after 3-month beclomethazone therapy.
