легочных артерий у больных с вторичной легочной гипертензией, обусловленной хронической обструктивной болезнью легких (ХОБЛ).
Проведено морфометрическое исследование легочных артерий в удаленной во время операции (хирургической редукции объема легких
(ХРОЛ)) легочной ткани 9 больных (1 женщина и 8 мужчин, средний возраст - 55,9 ± 9,6 лет) (ХОБЛ) тяжелого течения. Предоперационное исследование включало спирометрию, общую бодиплетизмографию, исследование диффузионной способности легких,
неинвазивную допплерэхокардиографическую оценку гемодинамики с измерением систолического давления в легочной артерии.
Все больные имели тяжелые обструктивные нарушения легочной вентиляции, выраженную гиперинфляцию легких, значительное
снижение диффузионной емкости легких, наличие альвеолярнокапиллярного блока. Систолическое давление в легочной артерии
составило 34,6 ± 4,7 mmHg. При морфометрии выявлено уменьшение диаметра и площади просвета сосудов, увеличение доли площади
мышечного слоя (в 2,2 раза) и доли площади интимы (в 8,1 раз) в структуре сосудистой стенки у больных ХОБЛ в отличие от группы
сравнения. Изменения структуры легочных артерий достоверно коррелировали как с величиной СДЛА, так и с функциональными
признаками гипервоздушности легких.
Полученные результаты показали, что несмотря на значительную перестройку легочных сосудов при тяжелых обструктивных
нарушениях легочная гипертензия развивается не у всех больных, причем повышение давления в легочной артерии носит умеренно
выраженных характер; развитие легочной гипертензии у больных с выраженными обструктивными нарушениями обусловлено
перестройкой ветвей легочных артерий, которая связана с эмфиземой легких.
