ХОБЛ тесно связана с коморбидными сердечно-сосудистыми заболеваниями (ССЗ), утяжеляющими ХОБЛ и повышающими риск смерти. Частота и распространенность ССЗ пи ХОБЛ составляет около 64 %. Примерно у 30 % пациентов с ХОБЛ наступает летальный исход от сердечно-сосудистых осложнений (ССО). Последние данные свидетельствуют о повышенном риске острого ИМ при тяжелом обострении ХОБЛ. Риск развития ССО увеличивается при тахикардии, повышении окислительного стресса и системном воспалении. По данным ряда исследований, для ранней диагностики ССО могут быть полезны не только методы визуализации, но и электрокардиография, а также анализ крови на биомаркеры. Несмотря на доказанные преимущества, некоторые препараты, обладающие способностью изменить прогноз при сердечно-сосудистой патологии, в частности кардиоселективные β-адреноблокаторы, у больных ХОБЛ используются недостаточно. Данный обзор посвящен некоторым аспектам обострения ХОБЛ и риска ССЗ.</p></abstract><trans-abstract xml:lang="en"><p>Chronic obstructive pulmonary disease (COPD) is a major public health problem worldwide. COPD is closely related to comorbid cardiovascular diseases, which contribute to the worsening of COPD and increased mortality. The frequency and prevalence of cardiovascular diseases in patients with COPD is about 64%. Approximately 30% of patients with COPD die from cardiovascular complications. Recent evidence suggests an increased risk of acute myocardial infarction in severe exacerbations of COPD. Tachycardia, increased oxidative stress and systemic inflammation in patients with COPD may contribute to an increased risk of developing cardiovascular complications. Several studies may be useful for early diagnosis, including electrocardiography, imaging methods, and biomarker blood tests. Some drugs that have changed the prognosis for cardiovascular disease, in particular, cardioselective beta-blockers, are underused in patients with COPD, despite proven benefits.
