Материалы и методы. Проанализированы данные прогноза у пациентов (n = 251) через 1 год после КШ. Сформированы 3 группы: 1-ю составили 62 (24,7 %) больных ИБС и сопутствующей хронической обструктивной болезнью легких (ХОБЛ); 2-ю – 65 (25,9 %) больных ИБС и хроническим бронхитом без обструктивных нарушений; 3-ю – 124 (49,4 %) пациента с изолированной ИБС. Критерием формирования групп явилось наличие хронической патологии легких и обструктивного нарушения вентиляционного типа. В течение 1 года анализировалось возникновение сердечно-сосудистых конечных точек. Результаты. Через 1 год наблюдения зарегистрировано закономерное уменьшение функционального класса (ФК) стенокардии и хронической сердечной недостаточности. Число нефатальных значительных сердечно-сосудистых событий не имело различий среди пациентов с ХОБЛ и без таковой. Зарегистрировано 7 (2,78 %) летальных исходов, основной причиной смерти во всех группах явилась сердечно-сосудистая патология. К неблагоприятным прогностическим факторам смертельного исхода были отнесены возраст, длительный (&gt; 3 лет) профессиональный стаж работы на химическом производстве, неполная реваскуляризация миокарда, индекс Тиффно и фракция выброса левого желудочка &lt; 40 %. Заключение. ХОБЛ является фактором неблагоприятного прогноза по частоте госпитализаций, связанных с сердечно-сосудистыми заболеваниями, а также более высокой летальностью.</p></abstract><trans-abstract xml:lang="en"><p>The first goal of this study was to analyze a relationship between 1-year outcomes and respiratory comorbidity in patients with coronary artery disease (CAD) underwent scheduled coronary artery bypass grafting (CABG); the second goal of the study was to determine the most significant predictors of fatal outcome in these patients. Methods. The study involved 251 patients underwent CABG. They were stratified according to having comorbid chronic obstructive (n = 62; 24.7%) or non-obstructive lung disease (n = 65; 25.9%) or not having chronic respiratory disease (n = 124; 49.4%). Cardiovascular events during 1 year were registered. Results. An expected decrease in functional classes of CAD and chronic heart failure (CHF) was noted over 1 year. Number of non-fatal significant cardiovascular events didn’t differ in patients with or without chronic obstructive pulmonary disease (COPD). Seven patients (2.78%) died during the year; the cause of death was cardiovascular disease in all cases. Factors predicting poor outcomes included age, occupational exposure to chemicals &gt; 3 years, incomplete myocardial revascularization, FEV1/FVC ratio, and left ventricular ejection fraction &lt; 40%. Conclusion.
